Founded in 2019 by Michelle Schmerge, DNP and Heather Smith, DPT, Remote Health exists to provide patients access to high-quality care, anywhere. The team is comprised of highly qualified nurse practitioners, nurses, and therapists, who offer primary care to patients 13 years and older. We envision a primary care that is on demand and proactive. Our patients can choose to be served in our clinics, from the comfort of their home, their place of employment, or anywhere they choose. We offer holistic, comprehensive primary care utilizing an interdisciplinary team. All our nurse practitioners are specialty certified including family medicine nurse practitioners that provide care across the lifespan as well as gerontology nurse practitioners specializing in the care of our senior patients. We are committed to allowing our older patients to age in place and deliver patient centered healthcare. Their approach is changing the healthcare model with outcomes of reduced hospitalizations and ER visits, as well as an increase in overall patient and employee satisfaction.

CEO & Co-Founder
Michelle Schmerge is a doctorally prepared nurse who has spent over a decade working as a front-line provider in post acute care. Dedicated to helping change the care paradigm, Dr. Schmerge has focused her recent work on strategies placing the patient at the center of care, driving clinical outcomes and value across the care continuum. Believing that exceptional patient care begins with a culture of wellness and resilience for clinicians, Michelle develops teams that value innovation and inclusion; creating opportunities for front-line team members to influence organizational strategy and improvement.
Dr. Schmerge is the CEO and Co-founder of Remote Health, a company committed to its mission of better health and care for older people. Additionally, Dr. Schmerge is a practicing adult nurse practitioner (NP) specializing in the care of an aging population. Prior to founding the company, Michelle served as Vice President of Population Health and Care Transformation for Advanced Home Care, overseeing clinical quality, development of strategic networks, and care model redesign. She has also served as the Director of Post Acute Care for Novant Health and started an independent medical clinic in Charlotte, NC.
Michelle received a Bachelor of Science degree in mathematics and a Master of Science in Nursing degree from Vanderbilt University and holds a Doctor of Nursing Practice degree from Yale University. Dr. Schmerge was named a 2019 Great 100 Nurse in the state of North Carolina and was the Joanne S. Stevens Political/Legislative Nurse of the Year for 2016. Michelle regularly volunteers with the North Carolina Nurses Association, advocating for the advancement of nursing.
Michelle, her husband, and two children live in Denver, NC where they enjoy sharing time on the lake with family and friends.
COO & Co-Founder
Heather Smith is a doctorally prepared physical therapist of fifteen years. She has practiced as a physical therapist across all settings of care and served patients across the lifespan. Dr. Smith has spent the last nine years of practice in the home health setting, as she has a passion for keeping aging adults safe and cared for in their homes and communities.
Dr. Smith is the COO and Co-founder of Remote Health, a company committed to its mission of better health and care for older people.
Most recently, Heather spent five years in healthcare leadership partnering with health systems, community partners and payers across multiple states in the US. She firmly believes that healthcare needs are local to a community and in partnership we can transform health outcomes and the way healthcare is delivered, all while keeping people at home or in the community to receive care.
Heather received her Bachelor of Science degree in Exercise and Sport Science from the University of North Carolina at Greensboro, a Master of Science degree in Physical Therapy from Winston-Salem State University and obtained her Doctorate of Physical Therapy from Shenandoah University. Heather was the recipient of the 2014 VNAA National Clinician of the Year award and 2019 AHHC of NC Leslie Carswell Leadership award.
Heather is an active member of the American Physical Therapy Association, including the home health and geriatrics sections.
She resides in Colfax, NC with her husband and son, along with their beagle, Blaze. She enjoys spending time outdoors, gardening, shopping and being with family and friends.
